In
1997, the Ministry of Community, Family and Children's Services created the Making
Services Work for People (MSWFP) policy document. Advisory panels were
created across the Region with the aim of
providing
recommendations for implementation of the MSWFP policy to the
Hamilton/Niagara
Regional
Office.
The Brant Advisory Panel recommended the formulation of a new
organization that would collaborate with the local community to develop a
“single point of access” for children’s and developmental services.
The Grand Opening of Contact Brant took place
on September 18, 2000 and the doors opened as of October 1, 2000, which
marked the beginning of an incredibly busy time for the Resource
Coordinators as the calls came steadily for assistance and support in
requesting service for individuals and /or their families.
The
primary functions of Contact Brant include:

Single point of access (intake, assessment
of need, recommendations and requests for service, waitlist coordination)

Linking families and individuals to
formal and informal supports (in Brant whenever possible)

Case planning including: Case
conferencing and case resolution

Residential Placement Advisory Committee
-
System
planning including identifying best practices, maintaining an extensive
data base of information and consulting with community partners
-
Passport - Administering the new MCSS funded Community Participation
Supports program for individuals with a developmental disability.
